Registrations

If you live within the Sellindge Surgery defined practice boundary you are welcome to register with us.  Please use the "Practice Boundary" tool, accessible from our home page, to check your postcode.

The links to the forms needed for registering with us are available below. Please ensure that both forms are completed and submitted correctly and allow TWO weeks for your registration to be processed.

Please note that any patient who moves outside of the Sellindge Surgery defined practice boundary will be removed from our list and will need to register at a new practice.

To gain access to online services, you should download an appropriate application and register your request with the Surgery in order to obtain a unique access code. (Contact the Surgery for further information).

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days.  After this period, you may be able to register as a temporary patient if you are still away from home.

You can be registered as a temporary patient for up to three months only. This will allow you to be on the local practice list and still remain a patient of your permanent GP.  After this you will need to register as a permanent patient.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
